To schedule a medical or dental appointment, please call
(260) 458-2641. Staff is available to schedule your appointment from 8-6 Monday and Wednesday and 8-5 Tuesday, Thursday, and Friday.

If you cannot come to your appointment, please call 24 hours before your appointment time to cancel.
