Substitute Breach Notification IMPORTANT INFORMATION FOR PATIENTS WHO CONTACTED THE CLINIC FROM JUNE 22, 2011, TO JULY 5, 2011. On August 30, 2011, Neighborhood Health Clinics sent letters to approximately 240 patients notifying them of a POTENTIAL compromise of their personally identifiable information during the time period of June 22, 2011, to July 5, 2011. Computer files, with at least two or more personally identifiable information (name, phone number, date of birth, limited medical information), were electronically removed from our premises without proper authorization. THE COMPUTER FILES DID NOT INCLUDE SOCIAL SECURITY NUMBERS. Neighborhood Health Clinics takes this potential data compromise very seriously and has taken corrective actions including notifying the proper local and federal authorities. While there is no evidence to suggest that personal data has been misused, it is Neighborhood Health Clinics' practice to notify those patients whose personally identifiable information may be compromised. If you contacted the Clinic to schedule a medical appointment during the time period of June 22, 2011, to July 5, 2011, your personally identifiable information (name, phone number, date of birth) may have been compromised. | Good Source | Supplies 10 to 19% of the daily value for a particular nutrient |
